    Please include a minimum of the following in your request:

  • Your Name
    		•  (and name of property owner, if different)
  • Contact Information
    		•  (such as mailing address, phone, fax, etc.)
  • Property Address
    		•  (if unavailable, provide the property tax id)
  • Property County
    		•  (i.e. Orange, Seminole, Volusia, Osceola, Brevard, Lake, Polk counties, etc.)
  • Type of Survey
    		•  (e.g. Boundary, Topographic, Foundation, As-Built, Route or Specific Purpose Survey, Tree Location, Rough Stake, Hub & Tack, Plot/Site Plan, Elevation Certificate, Flood Zone Determination, Letter of Map Amendment, Condo Exhibit)
